WHAT IS A COMMUNITY HEALTH CENTER?

The first community health center (CHC) opened its doors in 1965, with the mission of providing care to those who did not have access or had trouble affording healthcare.

Today, CHCs are at the forefront of providing primary care services, while also contributing to the individual communities they serve through health education and supportive programs.

Even though the health center model is more than 50 years old, CHCs continuously build on their mission and adapt to the changing needs of their communities by implementing innovative programs to expand access to care.

SLIDING FEE DISCOUNT SCALE

Primary Health Network accepts all patients, regardless of their insurance or financial status. Primary Health Network is a Federally Qualified Health Center, which in turn allows us to offer a wide range of services to patients through a discount scale.

We offer a Sliding Fee Discount Scale to all income-eligible uninsured or underinsured patients, based on annual household income for most of our services.
