Overview of Service

Consumers 60+ who have mental health issues or are developmentally disabled can also receive Care Management services through Mental Health or Intellectual Disabilities.

Core Components of the Service

Care Management for Mental Health

  • Blended Case Management: Most intensive, weekly, biweekly, monthly, bimonthly face to face home or community visits for goal setting, personal plan monitoring, referrals and resource information for individuals with mental illness for children and adults from 5 years old and up.
  • Resource Coordination: Face to face contact in home or community every 1 to 2 months for goal setting, personal plan monitoring, referrals and resource information for individuals with mental illness for children and adults.
  • Administrative Case Management: Least intensive contact. Minimal face - to face contact for yearly updates, system questions, referrals and resources.

Supports Coordination for Developmental Services

  • Supports Coordinator: Locates, monitors, and coordinates services for individuals with a developmental disability.
